The Educate & Prevent Initiative (EPI) utilizes community based approach to improve the health of underserved communities through health education, prevention and access to affordable care. Our model is focused on health education in preventing cardiovascular risk factors and diseases.
- Collaborate with local clinics and community organizations to understand the healthcare needs and disparities regarding cardiovascular risk factors in in local immigrant, refugee and low-income communities.
- Develop a community health project focused on simplifying health prevention of cardiovascular risk factors and engage affected community members.
- Implement the community health project at local partnering clinics and communities.
- Participate in community events and raise awareness about cardiovascular risk factors including high blood pressure, diabetes and hyperlipidemia.
